Chronic disease Management & care plans

Our GP’s can create either GP Management Plans (GPMP) or Team Care Arrangements (TCAs) for the management of chronic diseases.

Two different sorts of plans exist:

Your doctor might recommend a GPMP if you have a chronic (or terminal) medical condition.

Your doctor might also recommend TCAs if you need extensive care and need to see two or more other medical professionals for your therapy.

These Services are bulk billed but are subject to eligibility, you are only eligible for 1 Team Care Arrangement, GP Management Plan and Chronic Disease Management plan, per calendar year. Please check with the doctor or nurse about your eligibility.
